Book Description

A tale of ethical crisis and redemption, Standard of Care is one doctor's tumultuous journey as a senior executive in America's largest and most predatory hospital corporation. Weary of the tedium and diminishing returns of twenty-five years of private practice, Dr. Daniel Fazen becomes the new senior medical executive, the guardian of quality patient care, at Walnut Creek Memorial, his long-cherished community hospital. Without warning, eleven months later, Memorial is acquired by the Olympia Healthcare Corporation, the largest and, he knows, the most ruthless for-profit hospital conglomerate in America. At age fifty-five, with a taste for the good life and years of tuition ahead for his kids, Dan ponders a six figure incentive. With reservations-and rationalizations-he stays with Olympia. And so begins a downhill debacle...

5 out of 5 stars A fictional account with real-world feel.......2007-08-06

Full disclosure: I have the good fortune of knowing Dr. Kerns both personally and professionally. He is an amazing, talented and inspiring character --- and he has written a remarkable first novel.

I recently finished reading Jerome Groopman's excellent non-fiction "How Doctors Think". Groopman discusses medical errors often referencing the effects of the medical establishment. In "Standard of Care" Kerns personalizes that discussion through the conflicts of Dr. Dan Fazen. Faced with the very real-world challenge of being the physician he wants to be and the physician he is expected to be, Fazen reaches deep into himself and deep into his own past to find answers.

From the artfully crafted prologue to the powerful conclusion, this book will grip you. With characters you can care about, dialog that flows naturally, and some of the most important ethical questions of our times in the balance, you will not want to put this book down.

I thoroughly enjoyed it (twice) and highly recommend it.

5 out of 5 stars Medical Mess!.......2007-06-11

Dr. Kerns does an excellent job of merging the lives of two good people and the medical crisis this country faces today. Dr. Fazen (main character) is faced with tough temptations and tough choices. It is refreshing to read a book that deals with more than fictional psychotics, mass murderers and terrorists. The issues this book deals with are real and more frightening than any murder novel. I hope Dr. Kerns decides to write more and soon!! Before you spend $20.00 on the next Patterson rehash read this book. I think Dr. Kerns should give some insights as to how he feels we can fix the mess.

5 out of 5 stars Standard of Care and Raising Our Standards.......2007-06-08

Set on the stage of what seems to be a fairly typical large medical establishment, and against the backdrop of our country's deeply troubled and flawed healthcare system, David Kerns shares with us the story of Dan Fazen, a man who is caught between the world of corporate greed and doing what is right and honorable.

This is an inspiring story of one man's ethical crisis and ultimately his transformation.
It's about how one person, with ordinary concerns that many of us can relate to - navigates between what he believes in and the intense pressures he feels from a singled minded for-profit-over-all-else corporation.

It is my hope that people will buy and read this great book.
Whether real or fiction, stories about people having the courage to live lives of real integrity are always worthy of our attention.

I also hope that this book draws more attention to our country's broken system of healthcare. Clearly we need to raise our standards dramatically. The way our so-called healing institutions are run these days is as big a problem, as any threat of terrorism.

4 out of 5 stars Standard of Care.......2007-04-17

I just finished Standard of Care and must recommend this well-written, engaging book that conveys in fictional terms, the avalanching health care crisis. The book is about Dr. Dan Fazen, an especially likable character who is well fleshed out with seemingly minor details, but those colorful details tie into events that move the plot along. Those details or calming periods between heavy 'sentinal events' or medical catastrophes, are necessary in this narrative to keep it from getting too heavy. Kerns deft touch makes it well balanced. The locale and familar places to San Franciscans are a bonus for relating to the book, but that is by no means necessary for appreciating the book and the effort it took to develop a believable plot and carry it out. The crux of the conflict involves Dr. Dan's ethics of humanity expressed through his practice of medicine that are challenged when he is promoted to a high ranking hospital administrator. The hospital is acquired then in an aggressive buy-out and the ensuing cost-cutting places patients at risk. The plot takes on a national relevance because the antagonists, CEO Thomas Frost and his for-profit parent, the Olympia Healthcare Corporation, convey the coldness of the man and his corporation. This reference may be a thinly veiled version of HCA, the biggest, baddest for-profit hospital corp. in America, founded by the father and brother of Senator Bill Frist. The surprising ending finds Dan reclaiming his own moral ground while American health care teeters on the brink of collapse. As Bill Clinton said recently, the health care crisis is one of the the three largest crises the USA faces today. And good for Kerns to find it as fertile ground for the salvation of one doctor at least.

5 out of 5 stars Standard of Excellence!.......2007-04-13

David Kerns' novel rips the cover of respectability off of the medical-industrial complex and reveals its sordid underbelly in this cautionary tale. Standard of Care is disturbing and inspiring; horrifying and compelling; and ultimately a riviting and informative story. It will forever change your view of the American health care system and remind you that true heroes still exist.

    Book Description

Internal and external advocacy is a complex communication process, with many interwoven purposes, methods, and expected (or unexpected) outcomes. Judith Hoover and her contributors show what the advocacy processes are, using a fascinating set of case histories, and then analyze and evaluate them by means of rhetorical, cultural, critical, and argumentation theories. In doing so they blend organizational communication and classical rhetorical theory, and thus extend the concept of corporate advocacy into new areas of study. An important resource for teachers and students of communication theory and practice, and an unusual insight for corporate communication specialists. In fourteen case studies analyzed through three significant communication theory perspectives, Hoover and her contributors examine the concept of advocacy by looking at corporate rhetoric, corporate cultures, and the hidden sources of power inherent in both. We listen to the messages of corporate spokespersons such as Lee Iacocca. We observe the internal cultures of business and industry. We investigate the meanings of such terms as "Wall Street" and "consumerism." We broaden our view to include not only union advocacy, but also the role of language in the organizational distribution of power. By synthesizing these cases through yet a fourth perspective, the book not only extends the concept to recognize internal advocacy processes but also reveals the complexity of advocacy strategies that must be designed to accomplish multiple purposes and that must respond to multilayered and interconnected contexts.

Why campaign reform is crucial to the US economy and national security.

      Customer Reviews:

      5 out of 5 stars Magnificent!! A Masterpiece!!!.......2007-05-10

Jack Lohman shines a brilliant spotlight on the corruption and greed that has permeated our democracy, and exposes how corporate money influences the decisions of our elected officials in government. It's no secret that most of our politicians are under the control of corporate interests, and you'll find many well-documented examples on key issues exposed in this book. Only true campaign finance reform can save our political system from the corruption, which has infiltrated every aspect of our government. Mr. Lohman goes on to show how for only 15 taxpayer dollars per year, we could fully fund the election process. Also, by enacting a more fairly balanced election system such as IRV (Instant Runoff Voting), we could elect more majority-chosen candidates.

If you care about this nation, whether you're a politician, a soldier, a concerned citizen, or even a Rush Limbaugh sycophant, I urge you to read this book. The system is broken and it needs to be fixed. Too many jobs lost to oversea manufacturers, too many foreclosures on middle class homes, and too many neglected cities, in this cycle of corporate welfare generated by campaign contributions.

It's time to take our country back, and Mr. Lohman shows us a good number of ways we can do it.

5 out of 5 stars Joining the fight in Texas.......2007-01-13

Politicians - Owned and Operated by Corporate America is aptly titled. An honest book. The most refreshing thing that sets this book apart is the common sense solutions Jack Lohman offers. Obviously written from the viewpoint of a true patriot with insider knowledge. Covering a vast array of topics this is a handy reference on how to restore our government to the people. If you are reading this review you too must be concerned for the future of our country. I highly recommend this book. If it were a speaking engagement, the fee for one chapter would be a bundle and worth every penny.

5 out of 5 stars Know Who Works Against You.......2006-11-13

A concise and fact filled book with references that details the way Corporate America has co-opted our democratic system through campaign financing, lobbyists and money. The author not only shows who and how but offers ways to fix the system and return America to its democratic roots.

        Book Description

From lobbyists such as Jack Abramoff, to corporate executives, like Enron's Kenneth Lay and Jeffrey Skilling, recent scandals dealing with politics and government have focused only on men at the top. But do these high-profile men accurately represent the gendered make up of corporate-government in the United States?

In this first in-depth look at the changing face of corporate lobbying, Denise Benoit shows how women who have historically worked mostly in policy areas relating to "women's issues" such as welfare, family, and health have become increasingly influential as corporate lobbyists, specializing in what used to be considered "masculine" policy, such as taxes and defense. Benoit finds that this new crop of female lobbyists mobilize both masculinity and femininity in ways that create and maintain trusting, open, and strong relations with those in government, and at the same time help corporations to save and earn billions of dollars.

While the media focuses on the dubious behaviors of men at the top of business and government, this book shows that female corporate lobbyists are indeed one of the best kept secrets in Washington.

From the supplier: The National Consumers League considers itself as the premier consumer advocacy group in the US. Its projects and activities are sponsored and supported by large corporations and industrial organizations. Although its projects and conferences are sponsored by major corporations, the group refuses to divulge the specific amount given by the business organizations. The doubts expressed about the group's financial support results in suspicions on its genuine concern for consumer right advocacy.

From the supplier: Corporations have widened their political domain and further strengthened their political power with the advent of issues advertising. The corporate world found out that advocacy advertising pays off because it can elicit and influence public opinion, which can shape social policies and bring about reforms. Classic examples of highly successful issues advertising include 'Harry and Louise,' which defeated the health care plan of the Clinton administration. Paid advocacy is sure to play an important role in the coming federal elections.

From the supplier: The democratic nature of academic debate may be undermined by the way information has become increasingly available from corporate-sponsored databases. These databases were created primarily to serve business and legal entities, and their content threatens to limit the diversity of opinions since they typically provide texts only from mainstream publications. Thus, they would primarily serve corporate interests and limit the accessibility of other viewpoints.

                  Book Description

When you start a new job, you learn how things are done in the company, and you learn how they are complained about too. Unpopular Culture considers why people complain about their work culture and what impact those complaints have on their organizations. John Weeks based his study on long-term observations of the British Armstrong Bank in the United Kingdom. Not one person at this organization, he found, from the CEO down to the junior clerks, had anything good to say about its corporate culture. And yet, despite all the griping—and despite high-profile efforts at culture change—the way things were done never seemed fundamentally to alter. The organization was restructured, jobs redefined, and processes redesigned, but the complaining remained the same.

As Weeks demonstrates, this is because the everyday standards of behavior that regulate complaints curtail their effectiveness. Embarrass someone by complaining in a way that is too public or too pointed, and you will find your social standing diminished. Complain too loudly or too long, and your coworkers might see you as contrary. On the other hand, complain too little and you may be seen as too stiff or just too strange to be trusted. The rituals of complaint, Weeks shows, have powerful social functions.

5 out of 5 stars Company Culture: This is the way we do things here.......2004-02-24

As a student, the author was assigned to study a British retail bank on the understanding that when he was finished he would help the bank understand why morale there was so low. He was afraid it was going to be a boring assignment. Not a bit of it.

Hoping to get an in-depth understanding of the source of the problem, he asked to be sent to the worst department, a back-office Securities Centre where people complained non-stop. They complained about the bureaucracy, the procedures, the head office, the branches, the IT department, the staff shortages, the computer system, the pay, the bonuses, and the furniture. They complained about the way people always complained. They rolled their eyes at the union representative and at the "Rah Rah" videos sent from head office.

They sat gloomily while management explained why that quarter's huge profit was actually bad news that would mean that the hiring freeze would continue, if it didn't actually lead to layoffs. The bank's officers couldn't understand why the staff were always so negative.

The employees seemed to have taken a shine to the amiable young American and freely confided their opinions about everybody and everything. In fact, they seem to have been a rather jolly group. They understood exactly how far they could go and what the rules and rituals of complaint were. They managed to complain ceaselessly, while ensuring that nobody ever felt complained about. They were kind and self-deprecating and often quite content with their jobs.

In "Unpopular Culture", John Weeks examines the role of culture in an organization: how local cultures (such as that at the Securities Centre) relate to corporate culture. In particular, he observes how deliberate attempts to change the culture usually fail, while actual changes to the environment can have a profound effect.

Despite the slightly frivolous art work on the cover of the book, this is a thoughtful ethnography. I first heard of it when Dr Weeks was interviewed on the BBC Radio "Thinking Aloud" program that is, as of this review, still available on the web and well worth listening to.

                    1 out of 5 stars Inadequate.......2001-08-10

This book is woefully inadequate even for someone who already has a good idea what a living trust is about. The author presents forms with filled-in lines as examples that leave no options and there is almost no text to explain the essense of the forms so that one can tailor the entries. Even with another book at hand explaining the purpose of a trust and what it has to say, the forms in this book leave one without guidance for personalizing the trust. There may be some who feel these forms meet their needs but then one is left to wonder if they really know what they're doing. The author gives his phone no. and encourages users of his book to call him with questions but when I did, one response was something like, "Well, that's a lawyerly question". When I asked how to phrase something other than the 3 options given for the declaration of trust he offers, he said that all he offers is what is in the book.

In all, I find this book limited, narrow and lacking sufficient backup for writing one's own living trust.

This kit provides the tools needed to create your own living will ("advance directives")--those documents that name someone to act on your behalf, state your wishes about medical or surgical treatment if you become terminally ill or incapacitated, and give your instructions about CPR and organ donations. These important directives allow you to have some control in determining future medical treatment and will give your family the comfort of knowing and honoring your wishes. CD-ROM includes PDF forms.

I am now reading the 1995 edition: the information is excellent and clear. The author is a bit wordy, nevertheless. Readers may want to start with Appendix C ("Look, Chances are you may not have an estate tax problem in the first place"), then Chapter 2 ("The primary estate planning why you should have arevoable living trust: to avoid probate") to get a better idea of which other sections of the book will be most useful.
